While every precaution is taken for safety and good health, some recreational activities carry with them the inherent risk of personal injury.  I understand and accept these risks and agree to allow my child to participate in all activities. 

 I, the parent or guardian named above, authorize Resolute Kids Personnel to sign consent for medical treatment and to authorize any physician or hospital to provide medical assessment, treatment or procedures for the participant named above if necessary.

I, the parent or guardian named above, undertake and agree to indemnify and hold blameless the leadership, ministry volunteers, and the governing boards of Resolute Church from and against any loss, damage or injury suffered by the participant as a result of being part of the Break Away Day activities, as well as any medical treatment authorized by the supervising individual representing the church.  

I understand that my child may be visible in photos taken at Breakaway Day and used for promotional material.  I understand that no names or personal information will be published.
