I agree if I am late picking up my child I will be charged $5 for every 15 minutes that I am late.

I agree that there is a $25 non-refundable deposit required when Resolute Camp confirms my campers spot. The balance on payment is due on June 15. If after this time there is a balance owing, without a pre-arrangement made, my spot will be cancelled, and I will not be refunded the $25 deposit. Cancellations made after June 30, are 100% non-refundable.

I understand that Resolute Day Camp is a Christian camp where children will participate in Christian Bible lessons and discussions.

I agree that it is my responsibility to let Resolute Camp know ahead of time (at time of registration) any medical and behavioural issues, including special needs for the welfare of my child. If I fail to disclose this information with advance notice, then my child may not be able to participate in Resolute Day Camp for their safety and the safety of others.

Camper Code of Conduct Listed below are the expectations and behaviours we expect from all children who attend Resolute Day Camp. Campers will always be kind towards leaders and fellow campers. Campers will never use physical force towards others. Campers will use kind language towards others. Campers will never engage in bullying behaviour towards other campers. Campers will respect other camper’s property. Campers will listen and respect their camp leaders.

I understand that if my child is behaving in a way that is unsafe to themselves or others or requires one-on-one attention from a leader, my child will have a few minutes to follow the directions of the leader. I understand that if after a short period, my child hasn't followed the directions I will be called, and I must arrange for immediate (within 30 minutes) pick up of my camper.

I agree to be responsible for my child’s transportation home in the event of disciplinary action that warrants it necessary for the dismissal of my child from Resolute Day Camp upon being notified by an adult leader.

I authorize directors and adult leaders of Resolute Day Camp to obtain emergency medical treatment for my child at any time during these activities.

I understand that every effort will be made to notify parent/guardians first. If parents/guardians and the emergency contact cannot be reached, I give permission for my child to be evaluated, diagnosed, treated, and/or medicated by licensed medical personnel. In addition, I grant permission for the release of any medical information which I gave Resolute Day Camp to be given to medical personnel in case of illness. I/we the parent(s) or guardian(s) named above, authorize Resolute Day Camp adults to sign a consent for medical treatment and to authorize any physician or hospital to provide medical assessment, treatment or procedures for the participating child in the event of an emergency or where the parents/guardians and emergency contact cannot be contacted.

I agree that should it be deemed necessary for my child to return home due to medical reasons, I will arrange for an immediate pick up of my child.

I authorize Resolute Church to collect, use and disclose personal information as permitted by law, in connection with this consent to medical treatment, for the purposes necessary to obtain such treatment. I understand that personal information used, disclosed, secured or retained by Resolute Church will be held solely for the purposes for which they collected it and in accordance with the National Privacy Principles contained in the Personal Information Protection and Electronic Documents Act.
